29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).  Dry Dock 2, Port Entrance Stairwell: Section of a guard rail was missing on the wing wall exposing employees to a 9 foot 10 inch fall to the lower level.

Dry Dock 2, Portside, East Facing: The stairs and walking working surfaces were not maintained in good condition. Employees were exposed to tripping hazards.

29 CFR 1915.81(b)(1)(ii):  The employer did not ensure that each walkway was clear of tools, materials, equipment, and other objects that create a hazard for employees.  Dry Dock 2 Floor, Port Side of Eagle View: Employees were working in an area with badly damaged wooden planks that covered the drainage channels. The employees were exposed to tripping hazards.
