29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). Dry Dock 2, Port Entrance Stairwell: Section of a guard rail was missing on the wing wall exposing employees to a 9 foot 10 inch fall to the lower level.

Dry Dock 2, Portside, East Facing: The stairs and walking working surfaces were not maintained in good condition. Employees were exposed to tripping hazards.
