Prien Lake Rd., Lake Charles, LA, the employer failed to ensure an employee wore fall protection while installing framing to the exterior of a building from a Sky Jack SJ45T aerial lift basket. b) On or about July 23, 2019, at 1660 W. Prien Lake Rd., Lake Charles, LA, the employer failed to ensure an employee had a lanyard attached to the basket of a Sky Jack SJ45T aerial lift while installing framing to the exterior of a building.
