On or about October 13, 2021, on the south side of the site, an employee was cutting metal framing studs with a cut-off saw without proper eye and face protection.

1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner. On or about October 13, 2021, on the south side of the site, an employee was utilizing a mobile scaffold to install metal frames without properly locking the casters.
