At the jobsite located at 6120 Northwest Prairie View Road, Kansas City, Missouri 64151, employees were exposed to serious struck-by eye and face injuries. The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. The employee was working with assorted hand and power tools while conducting roofing work with the potential for eye injury without the use of eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At the jobsite located at 6120 Northwest Prairie View Road, Kansas City, Missouri 64151, employees were exposed to serious fall hazards. Employees were working at elevation without the proper use of guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards between 13 to 24 feet to the ground below.
