29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) Production Area- On or about July 8, 2019, the employer and the outside servicing personnel of Mazak Sales & Service Inc. did not inform one another of their lockout tagout procedures. b) Production Area- On or about July 25, 2019, the employer and the outside servicing personnel of Integrated Machinery Systems, Inc. did not inform one another of their lockout tagout procedures. c) Production Area- On or about July 26, 2019, the employer and the outside servicing personnel of Midwest Machinery Resources LLC dba Haas Factory Outlet Chicago LLC did not inform one another of their lockout tagout procedures. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
