Located at 137 Thrush Circle in Pickerington, Ohio: On or about July 25, 2019, the employer did not develop and maintain and effective safety and health program to protect employees from fall hazards while performing roofing work through training and hazard detection and correction onsite.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. Located at 137 Thrush Circle in Pickerington, Ohio: On or about July 25, 2019, employees were performing residential roofing work without protection from falls from the roof, exposing employees to a fall hazard of approximately twenty two foot. Miguel Dominguez was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1261589, citation number 1, item number 1, and was affirmed as a final order on 10/10/2017, with respect to a workplace located at 6400 Brandon Drive in Lewis Center, Ohio.
