29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded.  Meat department: The blade guard on a Hobart vertical band saw used to cut meat was stuck in the up position, exposing employees to amputation and laceration hazards.   The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1278225, citation number 2, item number 1 and was affirmed as a final order on 6/15/2018, with respect to a workplace located at Norfolk Commissary,  1588 Mall Drive, Norfolk, Virginia 23511.  Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1101252, citation number 1, item number 1 and was affirmed as a final order on 6/16/2016, with respect to a workplace located at Defense Agency Commissary Oceana, 1300 Oceana Blvd, Virginia Beach, VA 23456  Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1073789, citation number 1, item number 1 and was affirmed as a final order on 12/24/2015, with respect to a workplace located at Defense Agency Commissary Oceana, 1300 Oceana Blvd, Virginia Beach, VA 23456  Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1172530, citation number 1, item number 2, and was affirmed as a final order on 4/11/2017, with respect to a workplace located at Defense Commissary Agency, Selfrigde ANG, MI 48045.
