29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) 811 Park St. Atlanta -Court Yard, West Elevation Area - On or about 7/26/2019, and at times prior, the employer exposed employees to fall hazards of approximately 40 feet in that the scaffolds were not inspected prior to use at each work shift when employees were using scaffolds to install the windows at approximately height of 40 feet.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: a) 811 Park St. Atlanta Court Yard, West Elevation Area - On or about 7/26/2019, and at times prior, the employer exposed employees to the fall hazards of approximately 40 feet in that employees were not provided training for fall protection when they were using scaffolds and platforms to install the windows on the forth floor at approximately height of 40 feet.
