29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  The employer is failing to ensure that adequate fall protection is being used by employees when performing residential roofing.   On the jobsite located at 5501 152nd Street Omaha NE 68116 , the employer failed to ensure that the employees performing roofing activities were wearing fall protection while on the roof.  This was most recently documented on July 23, 2019.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employer failed to protect employees from fall hazards associated with ladders not being extended 3 feet above the eave, or 3 rungs to provide adequate hand holds when climbing onto or off the roof while utilizing the ladders.  This was most recently documented on July 23, 2019 at the jobsite located at 5501 152nd Street Omaha, NE 68116.
