29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 07/23/19, at 140 Eagle Feather Circle, in Spencerport, New York, employees working on a steep roof with a slope/pitch of 4-12, were not protected from falling 20 feet to ground level with fall protection.
