29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a.  On or about August 1, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun while conducting roofing operations did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.      a.  On or about August 1, 2019, at the above addressed jobsite, employees conducting roofing and fascia repair operations from ladder jack scaffolds, eight (8) feet four (4) inches and nine (9) feet two (2) inches above the ground, were not provided with access ladders, thereby exposing the employees to falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about August 1, 2019, at the above addressed jobsite, an employee was performing residential roofing operations without the use of personal fall protection, thereby exposing employee to fall hazards of ten (10) feet.    Westside Contractors, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1065581, citation number 1, item number 1 and was affirmed as a final order on July 8, 2015, with respect to a workplace located at 503 Birchwood, Glen Ellyn, IL.
