On or about August 1, 2019, at the above addressed jobsite, an employee using a pneumatic nail gun while conducting roofing operations did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about August 1, 2019, at the above addressed jobsite, employees conducting roofing and fascia repair operations from ladder jack scaffolds, eight (8) feet four (4) inches and nine (9) feet two (2) inches above the ground, were not provided with access ladders, thereby exposing the employees to falls.
