29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       a)  On January 7, 2020, an extension ladder used by employees to access and egress the roof was not long enough to have its side rails extended at least 3 feet above the roof eave. Westside Contractors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1420047, citation number 1, item number 1 and was affirmed as a final order on October 29, 2019, with respect to a workplace located at 1603 Devonshire Lane., Shorewood, Illinois 60404.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On January 7, 2020, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 26 feet above the ground.    Westside Contractors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1420047, citation number 2, item number 1 and was affirmed as a final order on October 29, 2019, with respect to a workplace located at 1603 Devonshire Lane., Shorewood, Illinois 60404.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
