On or about August 2, 2019, employees were exposed to a cave-in hazard while performing sewer line replacement at 3921 Medina Road, Fairlawn, Ohio 44333. The employees were working in a trench and the employer did not ensure they had a safe means of egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about August 2, 2019, employees were exposed to a struck by hazard while performing sewer line replacement at 3921 Medina Road, Fairlawn, Ohio 44333. The employees were working in an approximately 6.5 foot deep trench and the employer did not ensure that equipment and materials were placed at least two feet away from the edge.
