29 CFR 1915.159(b)(6)(iii): Personal fall arrest systems did not bring a falling employee to a complete stop or did not limit the maximum deceleration distance an employee travels to 3.5 feet (1.07 m).  F/T Kodiak Enterprise deck crane areas, on or about 31 July 2019 and times prior thereto; the employer did not ensure that proper fall protection was provided. Employees were using a Condor shock absorbing lanyard, serial number 4148774 with a maximum elongation of 48-inches to access the crane cable drum reel, 98-inches above the lower level. This condition exposed employees to a fall hazard.
