F/T American Dynasty - Administrative office; on or about 29 July 2019 and times prior thereto; the company executive did not ensure that the OSHA 300A Summary of Work-related Injuries and Illness Log for 2018 was accurate and complete, and/or deficiencies were identified and corrected at the end of the calendar year. Cases with days away from work (H) must be greater than zero if number of days away from work (K) is greater than zero. Total Recordable Cases (J) is different from Total of Injuries and Illnesses (M). This condition leads to inaccurate reporting of injuries and illnesses.
