OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by and struck-by hazards while lifting steel I-beams with an Harnischfeger - P&H overhead crane (serial number CH-31938-PP + 16303913A) equipped with below the hook beam tongs:     a) Sub Arc Line, 404 Sarah Furnace Road, Imler, PA 16655:  On or about July 29, 2019, beam tongs (built in house) designed and certified for vertical lifts only were used to side lift an I-beam weighing approximately 6803 pounds.   The I-beam was lifted off of the sub arc line table from a horizontal position (90 degrees from vertical) to a vertical position through the use of the beam tongs.  The crane operator and employees working near the Sub Arc line were exposed to struck-by and crushed-by hazards while the beam tongs were being used lift I-Beams from horizontal to vertical positions.           b) Sub Arc Line, 404 Sarah Furnace Road, Imler, PA 16655:  Observed on or about July 29, 2019, beam tongs (built in house) used to lift an I-beam weighing approximately 6803 pounds were not equipped with a tag showing the rated load.

29 CFR 1910.176(b): Storage of materials created a hazard:    a) Sub arc line, 404 Sarah Furnace Road, Imler, PA 16655:  On or about July 29, 2019, steel I-beams of various lengths and weights that were stored upright on the floor and on tables at workstations throughout the plant were not protected from being knocked over by the movement of steel beams, overhead crane rigging components, and other equipment.  An employee suffered serious injuries after an I-beam weighing approximately 6803 pounds that was placed near the sub arc table was knocked over onto the employee by beam tongs.             b) Jobsite, 404 Sarah Furnace Road, Imler, PA 16655:  On or about May 13, 2019, steel I-beams of various lengths and weights that were stored upright on the floor and on tables at workstations throughout the plant were not protected from being knocked over by the movement of steel beams, overhead crane rigging components, and other equipment.  An employee was injured after a steel beam was knocked over onto the employee by below the hook beam tongs.
