29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service.     a)     On or about July 13, 2019, The employer did not ensure that inspections were performed on the steel chain sling, used to lift raw material onto the Mitsubishi CO2 laser table, prior to use. Employees were exposed to crushing hazards in that the sling was missing the safety latch used to secure loads.

29 CFR 1910.303(g)(2)(i): Guarding of Live parts: Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  (A) By location in a room, vault, or similar enclosure that is accessible only to qualified persons;  (B) By suitable permanent, substantial partitions or screens so arranged so that only qualified persons will have access to the space within reach of the live parts. Any openings in such partitions or screens shall be so sized and located that persons are not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them;  (C) By placement on a suitable balcony, gallery, or platform so elevated and otherwise located as to prevent access by unqualified persons; or  (D) By elevation of 2.44 m (8.0 ft) or more above the floor or other working surface..    a)     On or about August 2, 2019, the employer did not ensure that the 230-volt electrical cabinet powering the Mitsubishi CO2 LASER table were guarded against accidental contact in that the cabinet panels were removed in an attempt to prevent overheating.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     a)     On or about: July 13, 2019, July 30, 2019 and October 1, 2019, an employee working on the Mitsubishi CO2 LASER table was exposed to a guarding hazard such as but not limited to: the point of operation while the cutting head is traveling toward the operator in that the machine's safety doors had been removed and the computer safety system was being bypassed to allow the equipment to operate.   b)     On or about July 13, 2019, July 30, 2019, and October 1, 2019, an employee was exposed to pinch points created by the perimeter frame and moving bed of Mitsubishi CO2 LASER table being operated in the bypassed.
