29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a) 1120 N Newport Rd, Colorado Springs: On or about August 2, 2019, workers were exposed to crushing and struck-by hazards when they were directed to perform work in an 8 to 10 feet deep excavation prior to a competent person performing an inspection and identifying hazards such as, but not limited to, improper protection from cave-ins and improperly located safe means of egress.
