29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: On or about 8/2/19, during tree trimming and removal activities located at 5679 Crown Ct. in Willoughby, Ohio, the employer failed to ensure that the unknown make and model number wire rope and synthetic rope slings, used for lifting tree limbs and trunks, were inspected for damage. a) The four (4) wire rope slings were missing identification labels, and had signs of damage to include kinking and bird caging, and b) The unknown make and model number synthetic rope sling was missing an identification label, and had signs of damage to include discoloration.
