29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. On or about August 6, 2019, at the above addressed jobsite, an employee working from an aerial lift did not have a lanyard connected to the aerial lift. The employee was thereby exposed to the hazards associated with a fall of approximately thirty (30) feet.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a. On or about August 6, 2019, at a above addressed jobsite, the wire rope midrail used in the guardrail system on the perimeter of the building was not maintained at 21 inches, thereby exposing employees to fall hazards.
