29 CFR 1910.147(a)(2)(ii)(B): An employee is required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle.   a) On or about July 30, 2018, in building 615 at Eglin AFB, the employer did not ensure that employees assigned to replace lamp's ballasts while standing on a step ladder, shut down a large industrial fan to prevent them from being struck by the fan blades spinning that were within the reach of the employee.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  a) On or about July 30, 2018, in building 615 at Eglin AFB, the Employer did not train the employees in the energy sources, how to locate and disconnect all sources of electric energy and methods necessary to render the electrical fixture safe.

29 CFR 1910.333(a)(1): The employer failed to ensure that live parts were deenergized before employees began to work on or near live parts.  a) On or about July 30, 2018, in building 615 at Eglin AFB, the employer did not ensure that employees assigned to conduct electrical work, including but not limited to replacing lamp's ballasts, de-energize the wiring providing 277 VAC power to a fluorescent lamps to prevent them from contact with energized electrical parts.

29 CFR 1910.333(c)(7): Portable ladders did not have non conductive siderails if they were used where the employee or the ladder could contact exposed energized parts.   a) On or about July 30, 2018, in building 615 at Eglin AFB, the employer did not ensure that employees assigned to conduct electrical work at heights, including but not limited to replacing lamp's ballasts, exposed to 277 VAC energized conductors use a ladder with non-conductive side rails.
