29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    Itasca, Illinois plant, Multivac 3 IP03 sandwich pouching line: on or about August 4, 2019, inspections of the control of hazardous energy (lockout) procedures and reviews with authorized employees were not conducted at least annually to identify and correct any deviations or procedural inadequacies. Maintenance personnel assigned to perform setup activities had their fingers exposed to moving machine parts.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (1) prepare for shut down by recognizing energy hazards and its controls,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device,  (5) render safe all potentially hazardous stored or residual energy,  (6) verify that isolation and de-energization of the machine or equipment have been accomplished.    Itasca, Illinois plant, Multivac 3 IP03 sandwich pouching line  On or about August 4, 2019, the employer did not ensure that maintenance personnel adjusting the tension on an indexing chain had isolated the hazardous energy sources prior to beginning work.  Because the equipment was not locked out, a technician had his fingers exposed to caught-in hazards from the unexpected movement of a chain and sprocket assembly while turning an adjustment screw.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.7(b)(3): How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.    Itasca, Illinois plant: On or about August 12, 2019, the employer did not ensure that an August 5, 2019 entry on their OSHA Form 300 Log of Work-Related Injuries and Illnesses include the number of days away from work.     There is no abatement documentation or certification required for this item.
