29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    On or about August 7, 2019, employees were not protected from falling through unguarded skylights, exposing employees to a fall of over 25 feet.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:     On or about August 7, 2019, employees performing roofing operations at a commercial building with a flat roof were not protected from a fall, exposing employees to a fall of over 25 feet.     ADVANCED ROOFING & WOODWORKING, INC.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1191347, citation number 2, item number 1 and was affirmed as a final order on January 11, 2017, with respect to a workplace located at 1125 E. St. Charles Lombard, IL 60148 .    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d)
