29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) Building Exterior: Employees were exposed to a 22 foot fall hazard while engaged in roofing activities from a ladder jack scaffold that was 24 feet long.. The end of the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, the ladder jack scaffold had an 8 foot overhang over the support, on or about 8/8/2019.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Building Exterior-left side: Employees were exposed to a fall hazard while accessing a ladder jack scaffold that was not provided with proper scaffold access. Employees were using the ladders of the ladder jack scaffold and going over the scaffold platform thus being exposed to a fall hazard, on or about 8/8/2019.
