29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process: 1) On August 5, 2019, and at prior times, the employer's P&ID was inaccurate in the following instances: a) The P&ID listed a vessel as AV315 but the vessel was labeled AV313 in the field; b) The P&ID listed a vessel as CT12 but the vessel was labeled CT 13 in the field; c) The P&ID listed a vessel as CT13 but the vessel was labeled CT 12 in the field; d) The P&ID listed an oil pot as KO 302B but the vessel did not have an ID tag in the field; e) The P&ID listed a vessel as PD302 but the vessel did not have an ID tag in the field; f) The P&ID listed two pressure relief valves, PD302-4 and PD302-8, but the valves did not have ID tags in the field; g) The P&ID listed a pressure relief valve as CT13-9 but the valve did not have an ID tag in the field; h) The P&ID listed two pressure relief valves, AV315-18 and AV315-19, but the valves did not have ID tags in the field.
