29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) LaLa US, Inc.: On or about 1/26/2022, and at times prior, employees were exposed to caught-in hazards that could result in lacerations, falls to the ground, and other injuries, from the slack in the plastic slat conveyor belt of the conveyors in the fluid filling room and sleeving room. The belt sags below the level of the guards on the conveyors. (b) LaLa US, Inc.: On or about 1/26/2022, and at times prior, the employer did not protect employees from injury in that the two-lane cottage cheese line was missing guarding near the operator terminal and where employees load lids into the machine. This condition exposed employees to caught-in, amputation, laceration hazards. (c) LaLa US, Inc.: On or about 1/26/2022, and at times prior, the employer did not protect employees from injury in that the three-lane cottage cheese line was missing guarding near the operator terminal and where employees load lids into the machine. This condition exposed employees to caught-in, amputation, laceration hazards. (d) LaLa US, Inc.: On or about 1/26/2022, and at times prior, the employer did not protect employees from injury in that the culture palletizer turn table area was not guarded on all sides. This condition exposed employees to caught in and struck by hazards. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). Lala US, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.212(a)(1)), which was contained in OSHA inspection number 1283354, citation number 1, item number 1, and was affirmed as a final order on 2/22/2018, with respect to a workplace located at 3801 Sinton Colorado Springs, CO 80907.
