29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  a). At the worksite - On or about August 7, 2019, the first level and side brackets of a fabricated frame scaffold were not fully planked or decked, thus exposing employees to a fall hazard up to approximately 12 feet.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation.  a). Front of the residential house - On or about August 7, 2019, there were four end frame legs on a fabricated frame scaffold that were not supported on base plates, thus exposing an employee to a fall hazard up to approximately 12 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  a). At the worksite - On or about August 7, 2019, a means of access (e.g. ladder) was not provided to access/egress the working levels of a fabricated frame scaffold, thus exposing employees to a fall hazard up to approximately 12 feet.

At the worksite - On or about August 7, 2019, employees performing overhead bricklaying operations from a fabricated frame scaffold were not protected from falling by the use of a guardrail system or personal fall arrest system, thus exposing employees to a fall hazard up to approximately 12 feet.
