29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Building 1: Employees were performing mixing operations with Portland cement. A flexible cord, which provided power to the mixer by a relocatable power tap, was identified with its plug pulled away from the jacket and wiring, exposing internal insulation and components. Employees were exposed to electrical shock hazards.
