29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Quality Construction, at 8200 W. 20th St., Greeley, CO 80634: On and before August 8, 2019, the exposing employer did not ensure employees were protected from injury, in that approximately 9 employees were engaged in re-roofing activities on a large apartment building, three stories high, without the use of a fall protection system. This condition exposed the employees to fall hazards of approximately 27 feet. Quality Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1244749, citation number 1, item number 1 and was affirmed as a final order on 9/27/17, with respect to a workplace located at 150 S. Ames St. Lakewood, CO 80222. Quality Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1406492, citation number 1, item number 1 and was affirmed as a final order on 7/9/19, with respect to a workplace located at 3637 W. 29th St., Greeley, CO 80634.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: (a) Quality Construction, at 8200 W. 20th St., Greeley, CO 80634: On and before August 8, 2019, the exposing employer did not ensure that employees were provided eye protection when power tools such as pneumatic nail guns were used during roofing operations. Condition exposed employees to the hazard of projectiles to the eyes.
