29 CFR 1910.119(d)(2)(i)(E): Information concerning the technology of the process did not include an evaluation of the consequences of deviations, including those affecting the safety and health of employees:    a) Plant - On or about 07/15/19 and at times prior thereto, the Process Safety Information (PSI) for the outdoor M-910 igniter operations did not include an evaluation of the consequences of deviations of the process conditions including (but not limited to) the temperature and humidity.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process.    a) Plant - On or about 07/15/19 and at times prior thereto the employer did not ensure that the hazards associated with the outdoor production and clean-up operations for the M-910 igniter process were addressed in the Process Hazard Analysis conducted on 06/15/16.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations in the M-910 igniter process and clean-up.    a) Plant - On or about 07/15/19 and at times prior thereto, the M-910 igniter production and clean-up operations were conducted outdoors, and the employer did not ensure that controlled conditions of temperature and humidity were maintained for high energy, sensitive materials.     b) Plant - On or about 07/15/19 and at times prior thereto, the M-910 igniter production and clean-up operations were conducted outdoors, and high energy sensitive materials in production quantities were transported by hand from the mixer to the processing table, and from the processing table to the grinder/separator.      c) Plant - On or about 07/15/19 and at times prior thereto, the igniter/tracer production and clean-up operations were conducted outdoors. The work table that supported the high energy, sensitive materials was damaged and unstable.      d) Plant - On or about 07/15/19 and at times prior thereto, the M-910 igniter production and clean-up operations were conducted outdoors. The floor surfaces in the work area were not maintained in smooth condition in order to prevent accumulation and facilitate the removal of hazardous materials.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.138(b): The employer did not base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified.    Plant - On or about 07/15/19 and at times prior thereto the ActivaRmr (Ansell) No 80-813 inherently flame resistant gloves selected by the employer did not protect workers from severe hand burns during the outdoor production/clean-up operations for the M-910 igniter process.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
