On or about July 30, 2019, at the address of 1010 New York Ave, Toledo, Ohio: The employer failed to report, within 24 hours, the amputation of an employee fingertip as a result of a workplace injury/incident. Mid-American Salt LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1299121, citation number 2, item number 1 and was affirmed as a final order on April 12, 2018, with respect to a workplace located at 20 Seaview Drive, Providence RI. 02905.
