29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)     Norfolk Naval Shipyard Building 171: On or about 08/13/2019, the Karmetal, model #500KSA, and serial #590-20 band-saw was not guarded to protection employees from a rotating worm gear.     b)     Norfolk Naval Shipyard Building 171: On or about 08/13/2019, multiple machine Lathes were not provided with a chuck guard thereby exposing employees to a possible hazard.    c)     Norfolk Naval Shipyard Building 171: On or about 08/13/2019, CNC lathes were used by bypassing the warning requirement to keep door shut during operation as a result employees were exposed to a possible hazard.  d)    Norfolk Naval Shipyard Building 171 section 400 Location J-2: On or about 08/14/2019, the Do-All vertical band saw #4215800114 used to cut metal the unused portion of the sawblade under the table was not guarded as a result exposed employees to a possible hazard.  e)     Norfolk Naval Shipyard Building 171 Toolmakers section D18: On or about 08/14/2019, the Do-All vertical band saw #06-457891277 used to cut metal the unused portion of the sawblade under the table was not guarded as a result exposed employees to a possible hazard.    f)    Norfolk Naval Shipyard Building 171 Nuclear Shop: On or about 08/14/2019, the Jet horizontal Band saw used to cut 6-inch pipe the unused portion of the sawblade was not guarded as a result exposed employee to a possible hazard.   g)     Nuclear Machinist Area Building 171: On or about 08/11/2019, the Birmingham Band saw #42150802146 used to cut mild steel the unused portion of the sawblade under the table was not guarded.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Notice 1 Item 2, 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: as required by 29 CFR 1960.8(b):  the yellow extension cords used to supply power to various appliances and a chop saw were not used in accordance with the listing and labeling in that one extension cord was plugged into another, thus exposing an employee to possible injuries. The outer jacket of the flexible cord was pulled away from the male attachment plug thus exposing the internal insulated wires.
