29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: Front side of the construction building: On or about August 14, 2019, and at times prior thereto, employees were exposed to a fall hazard while installing masonry and laying the blocks on the corner of a scaffold, without it being fully planked.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178: 29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d): Front side of the construction building: On or about August 14, 2019, and at times prior thereto, employees were exposed to a fall and ejection hazard, while the operator was not trained and evaluated on the safe operation of a Skytrak 6036 Telehandler.
