29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section:  a.  The employer did not furnish the abatement required for citations issued within 10 calendar days for Citation 1 Item 2 nor did they respond to the progress report letter sent December 10, 2019.  b.  The employer did not furnish the abatement required for citations issued within 10 calendar days for Citation 1 Item 3 nor did they respond to the progress report letter sent December 10, 2019.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   Location: Roof  On or about 8/15/19, the employer did not ensure employees working on a three story roof had their personal fall arrest system rigged to limit free fall to less than 6 feet. The employees had the life line attached directly to the back D ring on the harness without a rope grab which would have allowed them to fall more than 6 feet before system engaged to arrest the fall.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment.  Location: Roof Instance 1: On or about 8/15/19, employees working on a roof more than 6 feet above a lower level were not trained in the correct use of the personal fall arrest system they were using. The employees had the life line attached directly to the back D ring on the harness without a rope grab which would have allowed them to fall more than 6 feet before system engaged to arrest the fall.  Instance 2: On or about 8/15/19 an employee working on a roof more than 6 feet above a lower level were not trained in the correct use of the personal fall arrest system they were using. The employee was utilizing a personal fall arrest system had excessive slack in the body harness.
