29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  (a) Employee(s) conducting roof decking as part of rough framing work, stood on top of the 8/12 pitched roof of the structure to perform work without using fall protection.  Employees had not received training that would allow the employee(s) to recognize the related fall hazards and to implement and utilize the proper fall protection needed to minimize the fall hazard.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) Employee(s) conducting roof decking on an 8/12 pitch roof as part of rough framing work, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately twelve (12) feet.

29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a)  The portable  extension ladder erected on the South side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the roofs edge, and/or was not secured to the structure when it was not sufficiently extended.  (b)  The portable  extension ladder erected on the North west side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the roofs edge, and/or was not secured to the structure when it was not sufficiently extended.
