On or about 08/16/19, the storage room in the basement of 108-53 62nd Drive. Forest Hill, NY Employee was cutting a metal lock with a Makita 241 4B cut-off machine without a guard. ABATEMENT VERIFICATION IS REQUIRED.

Forest Hill, NY Two suspension scaffold was installed with single tieback in the angle. The employees working on two-point suspension scaffolds for exterior wall. ABATEMENT VERIFICATION IS REQUIRED.
