The job-made attachment was added to and used on a Clark forklift without the prior written approval of the forklift manufacturer; on or about June 3, 2020. Note: In addition to abatement certification, the employer must submit abatement documentation for this item in accordance with 29 CFR 1903.19.

The employer did not ensure that employees were wearing their seatbelt while operating a power industrial truck. An employee was crushed when the forklift tipped-over, on or about June 3, 2020. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
