29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed: (a)Yard - On August 14, 2019, the employer did not replace the badly kinked and rusted wire rope on the Coffing 3 ton hoist serial number WR3.103.XPB equipped on the Sessco 3 ton overhead crane. Identical deficiencies were noted to the 2018 annual crane inspection and the 2019 annual crane inspection. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.179(n)(2)(i): Hoist chain(s) or hoist rope(s) were not free from kinks or twists: (a) Yard - On August 14, 2019, the employer did not ensure that hoist chains used on the Coffing 3 ton serial number WR3.103.XPB hoist were free from kinks. Wire rope was badly kinked, dry and rusted and required replacement. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
