Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:  a) 6202 Germantown Ave., Philadelphia, PA 19144 - The employer did not ensure that employees who were walking on the face of formwork at heights up to approximately 9 feet above the lower level were protected from falling, thus exposing the employees to fall from height hazards.  This condition was observed on August 16, 2019.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  a) 6202 Germantown Ave., Philadelphia, PA 19144 - The employer did not ensure that employees who were walking on the top of concrete at a height  of approximately 11 feet above the lower level were protected from falling, thus exposing the employees to fall from height hazards.  This condition was observed on August 16, 2019.   Freedom Construction Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1243887, citation number 1, item number 1, group a,  and was affirmed as a final order on December 19, 2017, with respect to a workplace located at 1702 North Willington Street, Philadelphia, Pennsylvania.  Freedom Construction Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1239480, citation number 2, item number 1,  and was affirmed as a final order on December 28, 2017, with respect to a workplace located at 1735 Brandywine Street, Philadelphia, Pennsylvania.
