29 CFR 1910.217(b)(5)(ii): The operating levers on hand-tripped presses having more than one operating station shall be interlocked to prevent the tripping of the press except by the "concurrent" use of all levers.   In the saw department, the employer changed the configuration of the punch press and failed to inform the punch press operators of the modifications.  A foot pedal was added onto a two hand button operation for first shift without an interlock and was not returned back to the two hand configuration prior to second shift operating the punch press exposing the employees to amputation hazards.
