29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about March 25, 2021, and at times prior thereto, for employees exposed to amputation hazards while operating the SnoClip machine where guarding was not installed and used above the operation to prevent workers from placing their hands in the point of operation. (b) On or about April 6, 2021, and at times prior thereto, for employees exposed to amputation hazards while operating the SnoClip machine, where openings in the fixed guarding to include the ejection slide opening were large enough for a hand to be placed through the opening and into the point of operation.

29 CFR 1910.212(a)(3)(iii):Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: (a) On or about March 25, 2021 and at times prior thereto, the employer did not ensure special hand tools were provided or used to clear jams and material from the SnoClip machine operation, and (b) On or about March 25, 2021 and at times prior thereto, for employees exposed to amputation hazards while clearing product from a mechanical power press by reaching into the guarded area.

29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): On or about April 6, 2020, and at times prior thereto, for employees exposed to crushing and caught-in hazards while operating power press # 202; (a) where the adjustable guards were not configured so that they were not larger than what was necessary for the stock material, and (b) where employees stored material and reached into the machine area from the back of the machine to make adjustments. S-5! Manufacturing was previously cited for a violation of this occupational safety and health standard 1910.217(c)(2)(i)(a), which was contained in OSHA inspection number 1424682 , citation 1 item 2, and was affirmed as a final order on September 16. 2019, with respect to the workplace located at 500 West Highway Street in Iowa Park, Texas
