29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Location: South East side of building @ 999 William Canning Blvd. Fall River, MA On or about 8/21/19 an employee was exposed to a fall hazard of 12 feet while working from carpenter wall bracket scaffold where guardrail system was not used on all open sides or a personal fall arrest system was not provided.
