29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) On or about March 8, 2019, and at times prior, the employer failed to provide authorized employee training to employees that engaged in service and maintenance activities. As a result, the employer did not ensure that maintenance employees were knowledgeable and possessed the skills necessary to safely apply, use, and remove energy control devices when service and maintenance activities are performed.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in or was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about March 8, 2019, and at times prior, the employer did not ensure that employees implemented energy control procedures for the Orion packaging machine during the repair work being performed. All energy sources were not isolated prior to engaging in the maintenance or servicing activities. As a result, the remaining applicable energy control elements: machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from caught-in or amputation injury during machine servicing and maintenance activities.
