29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Lot #1 Fenwick Rd., Winchester, MA On or about August 21, 2019, employees were exposed to falls of up 12 feet, 3 1/2 inches to the ground during the construction of a wood frame house without fall protection. Instance A: Worker walking the top plate of a 2" x 6" wall 12 feet to the ground. Instance B: Workers exposed to falling stairway floor opening to the basement 9 feet to the gravel floor Instance C: Two front stairway footing form sleeves ~12 inch diameter by 79 inches deep that were not fully covered, not secured when installed so as to prevent accidental displacement, and not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard.
