29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  Location: First floor 1240 Washington Street Walpole, MA  Employees were exposed to falls of 10' through an open 45.5" W x 179" L on the first floor above a concrete basement floor and falls of 7.5' through an open 40" W x 85.5" L hole in a garage floor above a concrete basement floor.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   Location: Second floor deck, 1240 Washington Street Walpole, MA  Employees were not protected from fall hazards of up to 12-feet to the ground, while working on a second floor deck without personal fall arrest systems, a guardrail system, a safety net system, or an alternative fall protection measure.
