29 CFR 1910.159(c)(10): The minimum vertical clearance of 18 inches between automatic sprinkler systems and the material below was not maintained:  Building 2, Mechanical room: Employees maintain electrical power and back up systems throughout the work area. Inside the Mechanical Room, the sprinkler discharge pattern was obstructed by boxed picture frames stacked below the sprinkler head. Employees were exposed to fire and smoke inhalation hazards.

Employees are exposed to fall hazards 6 feet to a lower level.

29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive training on attachment adaptation, weight of the attachment, and use limitations;   Building 2, Power Pro Garage: Employees were using a forklift attachment to take photos and change light bulbs without being provided training specific to the work platform attachment. Employees were improperly using a ratchet strap to secure the work platform to the forklift mast. Employees are exposed to falls of 6 feet to a lower level.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  Building 2, Mechanical Room: Employees were exposed to electrical shock due to the strain relief (rubber sheathing) being pulled away from the coated electrical copper wire plug housing point. The APC backup power supply unit was found plugged into an 110 Volt electrical outlet.
