29 CFR 1910.1030(h)(1)(ii)(B): The employer did not establish and maintain an accurate record for each employee with occupational exposure which included a copy of the employee's hepatitis B vaccinations and any medical records relative to the employee's ability to receive vaccination as required by paragraph (f)(2) of the OSHA Bloodborne Pathogens Standard. a) Intake Department: The employer did not maintain hepatitis B vaccination records for employees that received the Hepatitis B vaccination series as observed on 8/23/19. Certification NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
