29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a. Production Area, Line 2: On or about August 23, 2019, and at times prior, the employer exposed employees to electric shock hazards, in that a 4 inch by 4 inch metal electrical box was used as a pendant receptacle and was used to provide power to a Triangle Engineering of Arkansas, Inc. 36 inch fan. b. Production Area, Office Shed by Line 2: On or about August 23, 2019, and at times prior, the employer exposed employees to electric shock hazards, in that a Underwriter's Laboratory listed and labeled relocatable power tap (RPT) supplying power to a TPI Corporation pedestal fan was not used in accordance with the guidelines of the listing and labeling agency where the RPT was attached to the building surfaces with cable ties.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. Production Area, Line 1: On or about August 23, and at times prior, the employer exposed employees to electric shock hazards, in that a TPI Corporation pedestal fan provided for personal cooling on the production lines was operated with the ground pin removed from the plug. b. Production Area, Line 2: On or about August 23, and at times prior, the employer exposed employees to electric shock hazards, in that a Triangle Engineering of Arkansas, Inc. 36 inch fan provided for personal cooling on the production lines was operated with the ground pin removed from the plug.
