29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 19, 2019, the employer failed to ensure that employees working in an excavation approximately 6 feet 10 inches deep were protected from cave-ins when speed shoring was used with only one hydraulic cylinder.
