29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:  a) On or about August 24, 2019, the employer did not ensure that each employee(s) engaged in scaffold activities 10 feet or more above lower levels were protected by guardrail systems. Employees of Medina's Masonry Corporation, were exposed to fall hazards of up to 16 feet while engaged in masonry activities on a residential building. 29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:  a) On or about August 24, 2019, workers of Medina's Masonry Corporation, were grinding stones that contained crystalline silica. The employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1. The area was observed to be covered with fine dust. When the workers were grinding on the stones, puffs of dust were visible as there was no vacuum or water attached to the machine. Employee exposure to respirable silica may result in an increased risk of developing serious silica-related diseases, including: Silicosis, lung cancer, chronic obstructive pulmonary disease (COPD), and kidney disease. No personal air monitoring had been conducted by the employer to assess compliance with the standard. The employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1.
